If the qualified patient's total loss from the waist, hips and thighs is less than 3.5 inches, the patient is eligible to receive up to another 6 ZERONA treatments at no charge."
To qualify for the Limited Guarantee
the following must occur:

Medical Professional & Patient must complete and sign the ZERONA Measurement and Limited Guarantee form. Patient must complete and sign The Authorization for Use and Disclosure of Protected Health Information form. |

The Patient must not miss any scheduled treatments. (All six treatments must be completed within a two-week period. The time between treatments cannot exceed 72 hours). |

The Patient must not have gained any weight. |

The Patient must return for Final Measurements approximately 7 days after the 6th treatment. |

The Patient must adhere to protocol of recommended steps for maximum results. |

The Limited Guarantee only applies for the standard ZERONA diode/laser placement over the waist, hip and thigh area. |
